PREREQUISITES
All students are required to attend an information session. During the session,
interested students will receive a program application packet, current program
information to include: tuition, lab fees, attendance dates, etc. and have
their questions answered.
Medical Assisting
Applicants must have 10th grade equivalency level in reading, math, language
on the TABE test, a high school diploma, or GED, and a completed application.
Application process includes evaluation, counseling, physical examination,
and orientation. Drug testing is required at registration, and random throughout
school year. Applicants must attend information session, in order to receive
application materials.
Massage Therapy
Entrance Requirements:
- Age: 18 years or older or turning 18 before course completion.
- A high school diploma or GED
Application requirements:
- Application form
- Copy of Driver's License
- Copy of High School Diploma or GED
- TABE scores (reading - 10.0, Math - 9.0, Language - 10.0
- Current physical examination
- Proof of Immunizations
- Current 2 step Tuberculosis testing
- Proof of immunity with positive blood titers for each of the following: Measles,
Mumps and Rubella OR the required Two MMR immunizations.
- Proof of immunity with positive blood titers for Varicella (Chicken Pox)
OR the required Two Varicella immunizations
- A Tetanus Booster within the last 10 years prior to admission
- Hepatitis B vaccines are STRONGLY RECOMMENDED. Students must comply with
OSHA Standards regarding Bloodborne Pathogens and Post Exposure protocols.
Students will receive instructions regarding this during the HEALTH CORE.
- Florida Department of Law Enforcement (FDLE) Background Check. A state background
check must be obtained by each applicant. Students with convictions may also
be required to obtain a waiver and committee permission to participate in clinical
experiences. Copy of check or money order must be submitted with application.
- Completed form stating that the applicant has had a massage from a licensed Massage Therapist.
Emergency Medical Services -
Paramedic & Emergency Medical Technician
Application requirements:
- Application form
- TABE test scores for Paramedic: reading - 11.0, Math - 10.0, and Language
- 11.0
- TABE score for EMT: reading - 10.0, Math: 10, language - 10.
- Copy of high school diploma or GED certificate
- Copy of Florida Driver's License
- Minimum of 18 years old

Clinical sites require a participating student to have an acceptable background
screening. To allow us to obtain a screening of your information, submit
the forms provided along with the other required documents to your career
counselor as part of the completed application. If you are accepted into
the program you will pay for the background check at the time of registration.
The cost for the background screening is $65.00 for established Florida
residents (those who have lived in Florida for the last 5 years,) if
you have not lived in Florida for the last 5 years, (non-established Florida
residents) the fee is $90.00. Payments must be made at registration
to allow time for all screening to be reported back to us in time for you
to go to clinicals on schedule. Felonies on your arrest records may
require you to submit additional documentation when applying for state
or national testing and deny you future employment.
- Copies of high school diploma, or equivalent
- Physical Examination and Immunizations. The physical must be dated within
one year of the course start date. A medical form is enclosed which should
be provided to the physician for completion. Immunization required include:
- Measles-Mumps-Rubella (MMR) - 2 doses required or positive titer
- Tetanus/Diphtheria (T/D) - Within the last 10 years
- PPD Skin Test (tuberculosis) - within the last year, TINE test not accepted
- Varicella (Chicken Pox) - 2 doses required or a positive titer
- Hepatitis B - highly recommended for this field
- Pregnancy: Students must provide a doctor's note stating that there are
no restrictions during the pregnancy.
Also required for Paramedic:
- Copy of current Florida EMT certificate
- 250 hours of documented field experience
- An interview with EMT paramedic Advisory Committee members
Also required for EMT
- copy of Health Care Provider CPR Card
Dental Assisting
The application
process includes the following: attendance at information sessions and orientation,
counseling,and physical exam.
First 20 completed application are accepted.
- Age: At least 18 years old
- Education: High school diploma or a GED certificate
- Completed Application form
- TABE test scores
(Minimum scores required: reading 10.0, Math 10:0, and language 10.0)
Current Physical Examination - The physical must be dated within one year
of the course start date
Immunizations:
- Measles-Mumps-Rubella (MMR) - 2 doses required or titer
- Tetanus/Diphtheria - within the last 10 years
- PPD Skin Test (tuberculosis) - 2 steps within the last year and current month
- Varicella (Chicken Pox) - 2 doses required, or titer
- Hepatitis B Vaccine - 3 doses (Highly Recommended)
Medical Biller/Coder
Application:
First 15 completed Applications are accepted
To be accepted in the program, you need to turn in the following items to Mrs.
Matthews:
- Completed Application form
- Copy of Diploma or GED
- copy of Driver's License
- FDLE Background Check
- TABE scores
Applicants must see a counselor and sign up for the TABE test. TABE scores
must be an 11.0 for reading and language and a 10.0 for math, for successful
completion of this program. The admission process also includes knowledge
of computers, and keyboards, completing an application and attending an orientation
session. Filling out the class registration form and paying fees take place
before the class. A background check must be done.
Student may request paperwork to release transcripts.
